Lab and X-Ray Reports
Most diagnostic tests are performed personally by your doctor here in the office. When necessary, outside tests such as x-rays and lab tests may be ordered. Most x-ray films and lab results are sent to the office within 24 hours of testing and are reviewed by the doctors on a daily basis. These results are usually discussed with you during your next office visit. If no follow-up visit is scheduled, you may call in for the results. In the event of any markedly abnormal lab or x-ray values, you will be notified as soon as results are evaluated.
Please notify the front desk if your HMO or other insurance requires you to use a specific lab or x-ray facility.

Medical Records
We ask that you fill out a questionnaire regarding your urinary problems upon your first visit to our office. This and any other information in your record is held confidentially and is the property of this practice. We do request your signature on a release form that will allow us to communicate back to your referring doctor. Your records will also be made available to you or anyone you designate by signing an additional release. These forms are available at the receptionist's desk. Please allow 3-5 business days for processing of medical records.

Phone Calls
We will always return your call, usually during office hours. As our in-house patients have our immediate attention during the day, sometimes your call will be returned in the evening after hospital rounds. We ask your assistance in letting our receptionist know if your problem is urgent so we can respond appropriately.

Hospitals
Both doctors practice at Ocala Regional Medical Center, Munroe Regional Medical Center, and West Marion Community Hospital. Dr. Edward King is also on active staff at the Villages Regional Medical Center and West Marion Community Hospital.
By combining the quality and up-to-date resources of area hospitals with full office services, the doctors endeavor to deliver an advanced standard of care in a traditional environment.

Payment and Fees
Medicare assignment is accepted by this practice. Co-payments and deductibles not covered automatically by Medicare and Medigap are ultimately the patient's responsibility; therefore, they are due when services are provided. We will give you statements necessary for you to file claims with any coinsurance you may have to reimburse you for this expense.
HMO and PPO patients must notify the receptionist of their enrollment before making an appointment to find out whether the doctors are participating in their plan. These patients should check with their insurance to see if there are restrictions regarding their choice of lab or x-ray facilities.
Private insurance will be billed for office visits when appropriate documentation is presented verifying insurance. We request that any co-payments or deductibles be paid at the time of service.
For all of our patients (Medicare and private)any balance not covered by insurance after 45 days of filing is considered the patient's responsibility. Please assist us by having correct insurance information on file.
Billing for Outside Services: When the doctors order tests for you that are performed by others, such as an x-ray, lab test, or microscopic evaluation of tissue samples, you will receive a bill from the people that performed the test. Our office does not handle the billing of other institutions, such as the hospital or other doctors and labs, even if our doctors ordered the test.
Let our insurance secretary know if special circumstances exist or if you have further questions regarding billing.

You can view and/or print the necessary patient forms for your appointment by following these easy instructions. In order to view and/or print the forms, you must have the Adobe Acrobat Reader, which you probably already have on your computer. If not, you can easily and quickly download a free version of Adobe Acrobat Reader by going to their website.
When viewing the forms in Adobe Acrobat Reader, the text might appear somewhat fuzzy; this is normal (the forms will print out clearly). To print, just click on the printer icon in the Adobe Reader toolbar above the document.
